The Problem We’re Solving Together

Diabetes is the most expensive, most preventable driver of poor health outcomes. Yet most people are struggling to put the pieces of medical care, practical tools, education, and support.

MettaH fills that gap.

Our 12-week virtual program integrates:

  • board-certified endocrinology,

  • evidence-based education

  • food as medicine tools,

  • and peer community support.

What a Partnership Looks Like

We don’t believe in a one-size-fits-all agreements. Partnerships are structured around your goals, your population, and your capacity. Depending on the partnership type, this can include:

  • Patient/member referral pathways with care coordination support

  • Co-branded programming for your community or workforce

  • Value-based contract structures, including PMPM arrangements

  • Joint data reporting and outcomes tracking

  • Community education and outreach through your existing channels

Every partnership starts with a conversation.

We’ll listen before we propose anything.
